Appointments
Please call our office at (562) 933-3141 for an appointment.
Please plan to arrive 15 minutes before each appointment to update any necessary forms. If you arrive late, your appointment could be rescheduled or cancelled.
Please note: Changes to appointments must be made at least 24 hours in advance.
New Patient Information Form
If your child is a new patient, please complete the new patient information form prior to your appointment, if possible, and bring it with you. This will allow us to attend to your child’s dental needs faster upon your arrival.
Insurance
We accept Medi-Cal Dental/Denti-Cal and some PPO insurance. We also welcome cash patients using a sliding-scale fee payment plan based on income for most treatments. Please contact our office to discuss insurance details.
Payment Options
For your convenience we accept cash and most major debit and credit cards. Payment is expected at the time services are performed. Feel free to contact our office if you want to discuss financial arrangements.
Visiting Our Long Beach Clinic
We are located at 455 E Columbia St, Suite 32, Long Beach, CA 90806. Our clinic is in the Buffum Medical Pavilion on the Miller Children’s and Women’s Hospital campus.

- There are several paid parking lots surrounding the clinic, and limited street parking is available.
Public Transportation
- Long Beach Transit Line 51 – Closest stop: Long Beach Boulevard & Columbia
- Metro 60 bus (Downtown Long Beach) – Closest stop: Long Beach Memorial Hospital
- Metro Blue Line – Closest stop: Willow St. Station (approximately 0.4 miles away)
